Wisconsin Death Records
In Wisconsin, death records are managed by the state’s Department of Health Services. These records may only be order by the deceased’s spouse, domestic partner, parent, child, sibling, or grandparent. You can also request records online by visiting a website which will provide them to you for a fee.
ID Required for WI Death Records
All requests for a death record must be accompanied by an accepted form of ID, such as:

The cost to search for a death record is $20.00. This amount is non-refundable if the requested information cannot be located. Additional copies of the same record are $3.00 each for orders placed at the same time.

Counter service is available from 8:00 a.m.-4:15 p.m Central Standard Time, Monday-Friday (except for legal holidays).
Regular and expedited service is available for orders placed in person. Regular orders for certified copies of death records are filled within two hours of acceptance if they are received before 2:00 p.m. Orders accepted after 2:00 p.m. will be filled by 10:00 .a.m. the next business day. If you request an uncertified copy of a death record, your request may take up to three months to complete.

Include a stamped, self-addressed envelope and the applicable fee with your request. You will also need to provide a photocopy of your ID in order for your request to be processed.
Requests for certified copies of death records are processed within 30 days. Orders for uncertified copies of death records will take up to 90 days to complete. No expedited service is available for orders placed by mail.

Expedited services for death record orders is available for fax orders. The document may be forwarded to you by regular mail or United Parcel Service (UPS) overnight delivery. All requests received in this manner are subject to an expedited fee of $20.00 in addition to the standard search fee. This amount must be paid by credit card, and a $6.00 credit card processing fee will be added to the order.
