Washington Death Records

If you are interested in ordering a Washington death record for a person who passed away recently (within the past month or two), you would order it from the local health jurisdiction. Death records dating from 1907 to the past two months can be ordered from the Center for Health Statistics. Washington Death Record Fees

In-person orders for death records cost $20.00 per certificate. Phone orders cost $20.00 per certificate ordered, plus a processing fee of $6.50 and $5.00 to expedite the order, for a total of $31.50 per death record. Expedited orders are subject to additional fees, as follows:

 

• – $18.30 for Express Mail
• – $18.50 for UPS Next Day
• – $25.50 for UPS Next Day to AK, HI and PR
• – $26.50 for UPS to Canada or Mexico
• – $37.50 for UPS International

Order a Washington Death Record in Person

Death records can be ordered in person on Monday-Friday between the hours of 8:00 a.m.-5 p.m. The Center for Health Statistics Office is located at:

If you choose this option, you can pay by check or money order payable to “Department of Health,” as well as cash, Visa or MasterCard. Orders received by 4:00 p.m. will be processed the same day.

To place an order for a Washington death record by phone, call (360) 236-4313. The hours of operation are Monday-Friday from 9:00 a.m.-4:30 p.m.

If you are requesting delivery by UPS Next Day Air, you will need to provide a street address for delivery. The courier company cannot deliver to a Post Office box using this delivery option.
