Vermont Death Records
Vermont death records for events which occurred within the last five years are maintained by the Department of Health. Earlier records are held by the Vermont State Archives and Records Administration. Death records may also be ordered online from our website linked to database which will supply the requested information for a fee.
In Vermont, death certificates must be signed by a physician. The doctor is also responsible for listing the official cause of death on the document. In many cases, the funeral director collects the information required to identify the deceased from his or her family. Once the information on the death certificate has been completed, it is filed with the town clerk. He or she sends it to the Health Department.
Fee for Vermont Death Records
There is no charge for a record search or verification that a death record is on file. The fee for each certified copy of a VT death record is $10.00. Make your check or money order payable to “Vermont Department of Health.”
Order Vermont Death Records by Mail
To order a death record from the Department of Health, and applicant will need to complete an application form (available online). The following information is required when requesting a copy of a record:
- – Name on Death Certificate
- – Sex
- – Date of death
- – City or town where death occurred
- – Date of birth
- – State of birth
- – Age at death
- – Name of spouse
You will also need to provide your full name, address and daytime phone number when making your request. The Department of Health will also want to know why you are requesting this information (settle the estate, apply for survivor benefits, family history, etc.).

Order Vermont Death Records Online
Online orders may be placed through the state’s Vital Records Request Service. You can order as many copies of a record as you like on a single order. Each copy costs $12.00, which includes the standard $10.00 fee and a $2.00 online service fee. Visa, MasterCard and Discover cards are accepted for online orders.
In most cases, an order placed online will be shipped out within five-seven business days by regular US mail. Expedited shipping is offered at an additional cost of $12.50 (flat rate), and this delivery option means that the order will be shipped overnight the same day it is filled.
If an order cannot be completed, you are entitled to a partial refund. You would only be responsible for paying the online service fee and the search fee for a VT death record which is ordered but not shipped.
Vermont death records can also be ordered from a website which offers this information to consumers for a fee. This may be the fastest and most convenient way to get a copy of a vital record sent to you.
