Utah Death Records
In Utah, death records are maintained by the state Department of Health’s Office of Vital Records and Statistics. If a particular death record is over 50 years old, it may be ordered from the Division of Archives & Records Service. These types of vital records may also be ordered online from a website which will provide them for a fee.
Who Can Order Utah Death Records
Access to UT death records is restricted to the following individuals:
- – Deceased’s immediate family (spouse, parent, child, sibling, grandparent, or grandchild)
- – Legal guardian of the deceased
- – Designated legal representative acting for the deceased’s estate or an immediate family member
- – Funeral director in charge of final arrangements
ID Required to Order Death Records
All applications for vital records must be accompanied by a valid form of identification. You can use one of the following for this purpose:
- – Driver’s License
- – Photo ID Card
- – School, College or University ID Card
- – Employment Card
- – US Military ID Card
- – Tribal ID Card
- – Alien Registration or Permanent Resident Card
- – U.S. Passport
- – Concealed Firearm Permit
- – U.S. Citizenship ID Card
If you don’t have one of the forms of ID listed above, you can use two of the following documents in its place:
- – Voter Registration Card
- – Work ID/Paycheck/W-2
- – Social Security Card
- – Marriage License
- – Divorce Decree
- – Probation Documents
- – Property Tax Receipt
- – Pilot License
- – Utility Bill
- – Medicaid Card
- – Court Order/Court Papers
- – Vehicle Registration
Fee to Order UT Death Records
The search fee to order a death record is $16.00. Additional copies of the same record cost $8.00 each.

Order Utah Death Records Online
The state does not accept online orders directly, but it has arranged for orders to be processed by an independent payment processing company. If you choose to place an order for a death record online, you will be charged a service fee along with the standard amount charged to conduct a search. An expedite fee is charged to process the order on a “Rush” basis.
Order UT Death Records by Phone
Phone orders for death records may be placed during regular business hours. An independent company processes these orders, which are paid for by credit card.
