Kentucky death records from 1911 to the present may be ordered from the Vital Statistics office. You can also place an order online by visiting a website which offers these records for a fee.

Under state law, information from vital records, including conducting a genealogical search or verifying whether a particular record is on file, cannot be released to the public at no charge. Anyone who wants to access this information must make a request in writing and pay the required search fee.
The amount charged for a search fee covers the cost of conducting a search and is non-refundable. It is non-refundable and the Vital Statistics office does not issue non-certified copies of Kentucky death certificates.
Information Required to Order a Kentucky Death Certificate
To order a death certificate in Kentucky, you will need to provide the following information:
- – Deceased full name
- – Month, day and year of death
- – County where death occurred
You will also need to indicate the number of copies of the death certificate you wish to order, your name and address so that the certificate can be sent to the right person. Include a daytime phone number where you can be reached from 8:00 a.m.-4:30 p.m. Eastern time.

The fee for each certified copy of a Kentucky death certificate is $6.00.
