If you would like to order in Indiana death record, they are available from the Indiana State Department of Health (ISDH) or from the health department in the county where the death occurred. Death records for people who passed away from October 1907 on are available from the ISDH. If you would like to order a death record from 1900-1907, you would contact the health department for the county where the death occurred.
The statement restricts who can order a copy of an Indiana death record to the following people:

- – Grandparents
- – Siblings
- – Aunts/uncles
- – Niece/nephew
- – Direct descendent
- – Funeral home/director (must be the one handling the body)
- – Other family members/cousins
Apply for an Indiana Death Record in Person
Indiana death records are available on a walk-in basis only from the health department where the event took place. You will need to show an accepted form of ID and improve your relationship to the deceased before the information will be released. Accepted forms of identification are:
- – Government issued drivers license or state ID card
- – Valid passport
- – Department of correction ID (issued within the past six months)
- – School ID (currently enrolled students only)
- – Work ID (current employer)
- – Veterans ID
If you don’t have one of the primary forms of ID listed above, you’ll be asked to produce two forms of secondary identification:
- – Employment letter (on company letterhead) confirming date of employment, employee’s address, and the managers name and signature
- – Original signed employment application (must still be employed by company)
- – Major credit card with signature
- – Previous year’s tax return (with signature and Social Security Number)
- – Signed mortgage or home rental agreement
- – Signed Social Security card
- – Probation documents or statement from probation officer on letterhead listing the person’s name and date of birth
- – Military Discharge documents
The average processing time for a death certificate is three to four weeks after the request has been received.
