Arizona Death Records
Arizona death records for deaths occurring after July of 2008 may be ordered from the State Office of Vital Records or the county office of the county where the death occurred. If the death occurred before July of 2008, you will need to make your request to the State Office of Vital Records in Phoenix. In Arizona, vital records are not public documents. The state’s Administrative Code R9–19–405 restricts the distribution of certified death certificates to the following individuals:
- – Spouse or immediate family members and in
- – Attorneys representing the deceased’s estate
- – Attorneys representing the deceased family members
- – Person having a legal interest in the deceased’s estate
- – Private investigators
- – Third party who has been granted permission from the surviving spouse
An Arizona death record may be provided for genealogical purposes, if the person requesting the document can provide proof of a relationship with the deceased with a birth or marriage certificate.
Arizona Death Records Fees
The cost to order an Arizona death record from the State Office of Vital Records is $20.00. county offices may charge a different amount for a death record, and you should contact the local office directly to ask about the current fee schedule.
Order Arizona Death Record in Person
You have the option of ordering an Arizona death record in person; however, same-day service is not available for certified copies. You can drop off your application between 8 a.m. and 4 p.m. and the document will be mailed to you. The Office of Vital Records is located at

When making your request in person, you will be required to show valid photo ID and provide proof of eligibility. Accepted payment methods for in person applications are cash or cashier’ checks, traveler’s checks, money orders, Visa or MasterCard. If you are paying by check or money order, it must be made payable to the Office of Vital Records.
Order Arizona Death Record by Mail
To order an Arizona death records by mail, send your request to either the State Office of Vital Records or the local county office. The mailing address for the state office is:

Be sure to include a photocopy of a government issued photo ID which shows your signature (or have your signature notarized). You will also need to provide proof that you are eligible to obtain the death record.
Payment may be made by cashier’s check or money order payable to the Office of Vital Records. If you wish to pay by credit card, include the card number and expiration date with your application.
