<p style="text-align: justify;">First, the agency is requiring proof of identity from each individual carrying out application for a decease certificate from the department. Requests may be established through providing a copy of state-granted photo ID, driver license, tribal ID, military ID, passport or Visa. If not, you can also present a copy of two of any of the following: Social Security Card, utility bill, bank statement and so on. There are currently three kinds of documents provided by the bureau particularly: Certification of the Facts of Death, usually containing social security number of the dead; Certification of Death specifying the cause of passing away; and lastly Informational Copy for informational purposes only.
