Arizona death records for deaths occurring after July of 2008 may be ordered from the State Office of Vital Records or the county office of the county where the death occurred. If the death occurred before July of 2008, you will need to make your request to the State Office of Vital Records in Phoenix. When making your request in person, you will be required to show valid photo ID and provide proof of eligibility. Accepted payment methods for in person applications are cash or cashier’ checks, traveler’s checks, money orders, Visa or MasterCard. If you are paying by check or money order, it must be made payable to the Office of Vital Records.

Be sure to include a photocopy of a government issued photo ID which shows your signature (or have your signature notarized). You will also need to provide proof that you are eligible to obtain the death record.
